What information do I hold?
As a Registered Psychotherapist in private practice, I collect some / all, of the following information: client / supervisee name, address, phone number, email address, date of birth and GP details. Information about supervisees’ clients are kept on paper and identified only by a code or single name.
What do I do with your information?
The information you give me when we contract to work together is stored securely as a paper record. Names, email addresses and phone numbers of current clients/ supervisees are stored securely on my computer and telephone, both of which are password protected. Only information which would be needed to contact you – for example about appointments – is stored in this way. Paper records are anonymised and kept securely in my office. Your information – including email address and number on my phone / computer – is destroyed three years after our work together has come to an end.
In line with good practice in the profession, names and phone numbers of current clients and supervisees are shared with a professional executor. This person would contact you in the event of my death or if I am unable to do this myself.
I will use your phone number, email or address to contact you regarding appointments and to reply to you if you make contact with me. I will never share your data with a third party unless legally obliged to. In this highly unlikely event, unless there was a legal prohibition, I would always inform you first farmacie-romania.com.
In the holding of this information I am legally required to seek your consent.
